Puppy Flow Participant Waiver and Risk Acknowledgement
Version: 18 October 2026 Revision 3
Operator: SERAFIMA TOMASHEVSKAIA, ABN 84 622 609 915, trading as Puppy Flow
Contact: team@puppyflow.com.au | Terms and Conditions: https://www.puppyflow.com.au/terms
Read this waiver and the Puppy Flow Terms and Conditions before accepting. It applies to the puppy yoga class, puppy interaction session or private event identified below (the Session). It includes a risk warning and release of liability.
Each adult must accept their own waiver. A parent or legal guardian must complete one for each participant under 18. A purchaser who is not attending accepts only the group booking responsibilities in section 9.

Participant means me if attending, or the child named above if I am their parent or legal guardian. A non-attending purchaser does not accept risks or make health declarations for other adults.
1 Health and suitability
To the best of my knowledge, the Participant is reasonably fit for the activities they choose and close contact with puppies. I will tell staff before the Session about any relevant allergy, injury, pregnancy, medical condition, disability, animal phobia or limitation affecting safe participation, including any change since completing this form. Detailed medical histories are not required.
The Session is not allergen-free; no breed is guaranteed to be hypoallergenic. I will seek medical advice where needed, particularly for animal allergies, pregnancy, a compromised immune system or immunosuppressive treatment. Staff do not provide medical advice or guarantee suitability following disclosure.
The Participant will not attend while contagious, required to isolate, or experiencing symptoms such as fever, persistent coughing, vomiting or diarrhoea. Yoga is optional: the Participant may rest, modify or stop an activity and will not attempt movements that cause pain or exceed their ability.
2 Risk warning and voluntary participation
WARNING: Physical activity and close interaction with live animals involve risks that cannot be completely eliminated. Puppies may behave unpredictably, even when supervised.
Risks include muscle strain, loss of balance, falls and aggravation of existing injuries or conditions; slipping or tripping over mats, equipment, toys, puppies, water or animal toileting; scratches, bites, licking, jumping and collisions; allergic reactions, asthma and exposure to animal dander, saliva, urine, faeces, parasites, bacteria, viruses or diseases that can pass between animals and humans; fear or emotional distress; and loss of or damage to belongings. Incidents may cause serious injury or illness and, exceptionally, permanent disability or death. Reasonable safety and hygiene measures reduce these risks but cannot eliminate them.
I understand these risks and voluntarily choose for the Participant to take part. I accept responsibility for assessing their abilities, following instructions and deciding whether to start or continue an activity. This acknowledgement does not excuse Puppy Flow from obligations imposed by law.
3 Safety hygiene and puppy welfare
The Participant must:
- follow reasonable directions from staff, the instructor and venue personnel, including hand hygiene, footwear and other biosecurity instructions;
- handle puppies calmly and gently, let them move away, and leave sleeping, eating, hiding, distressed or unwell puppies alone;
- not lift, carry, restrain, chase, wake or remove a puppy unless expressly permitted and supervised by staff;
- not pull a puppy’s ears, tail, coat or limbs, or press on its neck, spine, joints or abdomen;
- not feed puppies or bring animal food, treats, toys or other animal-related items without approval;
- avoid flash photography, loud noises and behaviour likely to distress puppies; and
- report injuries, bites, scratches, hazards or puppy welfare concerns immediately.
I will notify Puppy Flow before attendance of suspected or confirmed exposure of the Participant or a household animal to canine parvovirus or another highly contagious canine disease within the previous 14 days. Attendance then requires Puppy Flow’s approval following appropriate advice. No hygiene protocol eliminates all infection risks.
Staff may stop activities, rest or remove puppies, or stop a Participant’s involvement when reasonably necessary for health, safety or welfare. Refunds and other remedies remain subject to the Terms and Conditions and applicable law.
4 Children and supervision
The minimum age is 5. Participants aged 5–15 must have their own ticket and attend with a participating adult ticket-holder who actively supervises them throughout. Unless Puppy Flow approves otherwise, at least one participating adult is required for every two participants aged 5–15.
For a child, I confirm I am their parent or legal guardian and can provide the permissions in this form. I am responsible for arranging this supervision and ensuring the child follows instructions. Puppy Flow does not provide childcare. The supervising adult must remain present, respond immediately if the child is distressed or handles a puppy unsafely, and be able to contact the parent or guardian if absent. My acceptance records parental permissions and risk acknowledgements; it does not waive a child’s rights that cannot lawfully be waived.
5 First aid and emergencies
I authorise staff to provide or arrange reasonable first aid, contact emergency services where necessary and share relevant information with those assisting. Ambulance, medical and other third-party costs may be payable by me, except where Puppy Flow is legally responsible. This does not authorise non-emergency treatment or replace consent required by a treating health professional.
6 Assumption of risk and release of liability
I voluntarily accept the risks described in this waiver. To the fullest extent permitted by applicable law, I release Puppy Flow, its owner, employees, instructors, contractors, agents, puppy providers, handlers and participating venue operators from claims, liabilities, losses, injuries, damages and expenses arising from my participation in the Session, including those caused by their negligence.
Nothing in this waiver excludes, restricts or modifies any right, guarantee or remedy under the Australian Consumer Law or other applicable law that cannot lawfully be excluded, restricted or modified. This release does not apply to fraud, reckless conduct or any other liability that cannot lawfully be excluded.
The Participant will take reasonable care of belongings. This does not exclude any liability Puppy Flow has under applicable law for their loss or damage.
7 Photography and video
Media arrangements are set out in the Terms and Conditions. Accepting this waiver does not grant new or broader media permission. Adults may decline promotional photography before or during the Session without affecting participation.
Before intentionally photographing or filming a child, staff must obtain explicit parental or guardian permission. Permission to capture an image and permission to publish it on Puppy Flow’s website or social media must be confirmed separately from this waiver. The child’s own objection will also be respected.
Incidental background capture does not override refusal of consent. Puppy Flow will take reasonable steps to avoid publishing identifiable images of anyone who has declined, including cropping, blurring or excluding images.
8 Personal information
Information is collected to identify participants, record acceptance, administer the Session, manage safety and respond to incidents or related legal and insurance matters. It may be shared with authorised personnel, emergency services, insurers or professional advisers where necessary and permitted by law.
Only relevant health information is needed; sensitive concerns may be discussed privately with staff. Contact team@puppyflow.com.au with questions. This waiver does not subscribe anyone to marketing. Further information is available in Puppy Flow’s Privacy Policy: https://www.puppyflow.com.au/privacy-policy.
9 Group bookings
If booking for others, I will share this waiver and the Terms and Conditions with them. Each adult must personally accept their waiver; a parent or legal guardian must complete one for each child. Anyone who has not done so must use the link or QR code at check-in before participating. My booking or acceptance does not replace another adult’s waiver.
